One additional identity document (preferably with photo) i.e. Work badge,
student ID etc.
Social security card (No copies will be accepted)

You may apply for a driver’s license once you are 16 and have completed an approved driver education course, have a financial liability form signed, furnish positive identification and proof of residency and complete medical, vision and skills testing.
All individuals who have never been licensed to drive a motor vehicle must complete an approved driver education course before being licensed in Utah. A certificate of completion of the course must be furnished to the examiner at the time application is made. Remember that this certificate of completion is not a license to drive. Effective July 1, 2003, local school districts may allow a student to complete the classroom instruction portion of driver education in the public schools, through a home study program, or electronic high school, and allow the student to complete the behind-the-wheel portion of driver education with a private provider. All individuals under 18 years of age applying for a driver license for the first time must complete 40 hours of driving a motor vehicle, of which at least ten hours must be during night hours. The hours of driving in a driver education course may be counted as part of this requirement. The 40 hours of driving a motor vehicle may include up to five hours completed by driving simulation practice on a fully interactive driving simulation device.
Other requirements and additional information may be obtained in the Utah Driver’s Handbook and you should review the medical requirements as well.
